Before beginning, please prepare: 

1. Passport Information

2. Medical Information

3. A Digital Passport-Style Photo in JPG or PNG format

4. Please use a computer, not a cell phone, to complete the application. 

We do not accept any hand written applications. 

At the end of the application, you will be asked to print a form. This form must be printed, completed, signed, and mailed with a check for the $200 application fee.

YOUR APPLICATION WILL NOT BE COMPLETE UNTIL THIS FORM IS RECEIVED.
