Before beginning, please prepare: 

1. Passport Information
2. Medical Information
3. A Scan of your Passport
4. A Digital Passport-Style Photo

(Headshot Only) in JPG or PNG format
5. List of your immediate friends that you want to be on a tour with

(include their wishlist number)

WE DO NOT ACCEPT ANY HAND WRITTEN APPLICATIONS 

-Please use a computer, not a cell phone,

to complete the application. 

At the end of the application, you will be asked to print a form. This form must be printed, completed, signed, and mailed with a check for the $200 registration fee.

 

YOUR APPLICATION WILL NOT BE COMPLETE UNTIL THIS FORM IS RECEIVED.
