Tour #3 Application

Application Form & Behavior Code - Terms and Agreements see below

  • Before you can sign up, you must do the following;

  • Please follow directions carefully or your application will NOT be accepted!

​​

  • You must join the "Wish List"  (Free e-mail list so we can keep you updated). 

  • Trip space is limited and will be sold in the same order that you have joined the "Wish List"  

  • If your friends joined the "Wish List"  weeks after you joined, they may NOT be able to be on the same package or in your room. 

  • Meet with all your friends and their parents and decide which trip number and which lodging package you will pick.
            Note: You cannot visit friends in the other hotels. They are off-limits. You will see them each day!
            Read over the accommodation page and type up a rooming list with your friends.

  • ​Your rooming list is ONLY a request. 

  • No guarantees since your lodging package may be sold out or your friends signed up too late.

Step #1 

Check the expiration date of your passport.

​

European Union rules state that the passport MUST be VALID for 6 months AFTER your return date to the United States.


Tour # 2  – Your Passport must be valid till at least February 6, 2025

Step #2
 

If passport is valid...​

  • Make a full size COLOR photocopy of the information page and enclose
    it with your application.

​​

  • Please do Not enlarge or shrink the copy. 

​

If passport is NOT valid...​

  • Go to www.travel.state.gov and find a processing center near you.  
           Warning: It is taking up to 6 - 10 weeks.
         Do Not expedite!  Save your money
          for Europe.

  • Apply for one Today!

  • Many processing centers require an appointment - But several allow walk-ins. Call them to verify.


  • On the application form where it has Passport #____   Please type in “Being Renewed”

Step #3 

Staple a NEW 2"x2" Passport Photo to the application form in the  top righthand corner​

  • No photo and your application will be denied.
           Go to a     Staples, drugstore photo     
          department or FedEx and get your
          photo today.

  • Make it a cute one because everyone will see it!

  • Please print your name on the back and staple on the edge of your photo, NOT on your face.

Step #4
 

Download the Application 

  • Use a computer, not a phone!

  • TYPE in all of your Information in CAPITAL LETTERS,  (except the signatures)  

  • BEFORE you print it on single sided pages.  Handwritten forms will NOT be
    accepted.

  •  Please print on single sided pages.
            Sit down with your parents and
            read over the forms and both of
            you need to sign the forms.

Step #6
 

Enclose the Following Information: 

  • 1 NEW passport photo (2"x2") stapled to application form  in the upper right-hand corner. No staples on your face! 

  • 1 COLOR photocopy of your passport information page/ your photo page with dates and numbers 

  • 1 Application Form signed and dated

  • 7 pages of Terms and Conditions. Initial each page and sign the last page with your parents 

  • 1 Medical Emergency Form

  • 1 Behavior Code, signed by both you and your parents 

  • Add a piece of paper listing your roommates and a master list of close friends that you want to travel with. 

  • 1 Personal Check, Cashier's Check, or Cash for $95.00, made payable to BUZZ'S ADVENTURE TOURS
    Sorry, NO credit cards or electronic payments.
